Salary : $105,000 - $110,000, plus a $15,000 sign-on bonus.

Location : Manhattan, NY (Roles available in Queens and the Bronx)

Schedule : Friday, Saturday, and Sunday - 13-hour shifts

Position Overview : As a Weekend Field Nurse / Coordinator of Care, you will play a critical role in the home care team, overseeing all aspects of patient care. This is a stable, full-time, permanent position that allows for flexibility in your schedule while making a meaningful difference in patients' lives.

Key Responsibilities :

• Conduct comprehensive home care assessments to determine patient eligibility and develop tailored care plans.
• Deliver skilled nursing services, including wound care, medication management, and disease education.
• Effectively communicate and coordinate with patients, families, medical providers, and interdisciplinary teams to ensure optimal care.
• Maintain precise documentation in HCHB clinical software, adhering to required timelines.
• Supervise and orient home health aide personnel as necessary to ensure quality care.
• Identify needs for additional services (e.g., PT, OT, MSW, speech therapy) and coordinate those services accordingly.
• Monitor patient progress, adjust care plans as necessary, and actively participate in case conferences.
• Ensure compliance with infection control standards and agency protocols to maintain a safe environment for patients.
