A company is looking for a Clerical Territory Patient Admissions Coordinator.
Key Responsibilities
Collaborates with referral sources, sales teams, and clients to gather information for quality referrals
Documents referral-related information and actions in the medical record accurately and timely
Ensures prompt attention to referrals from various platforms and follows up with clients and caregivers as needed
Required Qualifications, Training, and Education
High School diploma or GED
Minimum of one year experience in home care, hospice, or personal care services
Prior experience with Electronic Medical Records (EMR), HCHB preferred
Experience dealing with various healthcare payors, such as Medicare and Medicaid, preferred
