Adams Farm Living and Rehabilitation, part of the Century Care family, is currently hiring! Our facility provides residents with a full range of long-term care and rehabilitation services, along with a variety of engaging social and recreational activities. RN MDS Coordinator

The Care / MDS Coordinator is responsible for timely and accurate completion, coordination, recording, updating, and monitoring of all Minimum Data Set (MDS) documentation required by state and other appropriate authorities.

Specific responsibilities include:

• Provide clinical and administrative support to the Director of Nursing Services
• Regulate and maintain the MDS/PPS program and completion schedule
• Develop and coordinate appropriate nursing care plans for all residents
• Evaluate effectiveness of nursing care plans, and update MDS/RAI process within completion guidelines
• Works in collaboration with the Interdisciplinary Team to assess the needs of the resident
• Monitors MDS and care plan documentation for all residents. Ensures documentation is present in the medical record to support MDS coding.

The ideal candidate will possess the following:

• Required - Active and unencumbered NC License as an RN
• Prior MDS and Case Mix experience is preferred.
• Case management experience is preferred
• Experience with Electronic Health Records, PointClickCare Preferred
• Ability to communicate effectively with residents and their family members, and at all levels of the organization
• Ability to organize and prioritize
• Ability to maintain confidentiality
• Ability to be accurate, concise and detail oriented
• Knowledge of RAI requirements is preferred
