Responsibilities

• Provide skilled nursing care to patients in their homes according to care plans and physician orders.
• Assess and monitor patient conditions, reporting changes and concerns promptly.
• Administer medications and treatments as prescribed, maintaining accurate documentation.
• Educate patients and their families on managing illnesses and maintaining health.
• Collaborate with healthcare team members to coordinate patient care.
• Adhere to infection control procedures and safety protocols.
• Maintain patient confidentiality and comply with all regulatory requirements.

Requirements

• Current and valid Registered Nurse license in the state of Michigan.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Basic computer skills and comfort with electronic health records.
• Reliable transportation to travel to patient homes.
