Schedule : Monday – Friday, 8 : 00am – 4 : 30pm (8-hour day shifts); weekends as needed / n

Hours per Week : 40 / n

Estimated Weekly Salary : $2,370 - $2,430 / n

/ n / nRegistered Nurse Case Manager Qualifications / n

/ n

Active RN license in New York State (NY RN license required) / n

Minimum 1 year of clinical nursing experience in a hospital or acute care setting / n

At least 9 months of home health care or home care nursing experience preferred / n

Certified Home Health Care experience required / n

OASIS documentation experience required / n

Telemetry nursing experience required / n

BLS certification (any provider accepted) / n

EPIC or Netsmart EMR experience preferred / n

Current NYS driver's license / n

Strong clinical assessment, critical thinking, interviewing, and communication skills / n

Ability to work independently, exercise initiative, and adapt to changing assignments / n

/ n / nRN Case Manager Responsibilities / n

/ n

Develop and revise individualized treatment and discharge plans in collaboration with patients, families, physicians, and interdisciplinary care teams / n

Complete and submit OASIS assessments, transfers, resumptions of care, discharges, and significant change in condition documentation within agency timeframes / n

Process physician orders, notify providers of patient needs and changes, and complete certification / recertification orders and discharge summaries / n

Deliver specialized nursing care, preventive and rehabilitative strategies, and patient / family education in the home health setting / n

Supervise LPNs and coordinate the quality of patient care services / n

Maintain compliance with New York State and federal regulations and agency policies / n

Participate in patient care conferences, peer review, and quality improvement initiatives / n

Promote a respectful, confidential, and supportive environment for patients and staff / n

Demonstrate flexibility in patient visits and ensure timely problem resolution / n

Support care management philosophy focused on collaborative, patient-centered planning / n
