Incumbent serves as a Social Worker (Program Coordinator) and is administratively responsible for a clinical program providing treatment to patients in the Housing and Urban Development / VA Supportive Housing program. Basic Requirements : Drivers License : Must hold valid, unrestricted driver's license. Education : Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure : Must be licensed or certified by a state at the advanced practice level and must be able to provide supervision for licensure. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum / course of study and internship, or equivalent supervised professional experience. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. The incumbent will act as a consultant for HUD-VASH social workers who serve Veterans with complex substance use disorders. Performs work related to the review, monitoring, analysis, planning and assessment of the HUD / VASH program. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Performs clinical social work functions and specialized clinical social work functions. Independently conducts psychosocial assessments. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Identifies high-risk patients and provides clinical care and services. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Establishes and maintains effective therapeutic relationships with veterans and their families. Provides consultation services to other staff regarding the psychosocial needs and substance use disorder treatment needs of veterans and the impact of psychosocial problems and substance use on health care and adherence to treatment plan. General knowledge of veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension.
