Outpatient Physical Therapist (PT) - Jacksonville Float, PRN

Job Description

Brooks Rehabilitation is seeking a skilled and compassionate Physical Therapist to join our Outpatient team. This role is ideal for professionals who are dedicated to providing high-quality patient care in a dynamic, patient-centered environment. You will have the opportunity to work in a collaborative environment with experts in the field to help facilitate personal and professional growth and provide superior patient care.

Location: Our outpatient float team covers our entire Jacksonville and surrounding areas in Duval, Clay, St. Johns and Nassau counties. PRN Floats will select one of the primary service areas where our team will work to place you the majority of the time. Placement will not be made outside your region without prior approval.

Primary Service areas include:

• Central/East Jacksonville
• South Jacksonville/St. Johns County
• West Jacksonville/Clay County
• North Jacksonville/Nassau County

Department: Outpatient - Float Team

Hours: M-F; PRN between 7A-7P

Responsibilities:

• Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs
• Deliver high quality patient care for multiple rehabilitation diagnoses
• Provide a positive patient experience through patient engagement and progressive treatment
• Effectively communicate both verbally and written
• Promote evidence-based patient care

Qualifications:

• Current PT license in state of Florida
• Current hands-on CPR/BLS Certification
• Knowledge of federal and state professional requirements
