JOB DESCRIPTION:

SUBMISSION REQUIREMENTS

• Must have clean background and malpractice
• Must have ATLS
• Must have AZ license and DEA
• Must have ACNP or certification
• 1st assist & trauma experience preferred
• At least 1 year of experience in Trauma/ED and ICU Submission Requirements

AS A REMINDER:

• All candidates must have met the minimum shift availability listed on the job.
• CV (Curriculum Vitae) must have all dates in MM/YY to MM/YY format
• Work History must be in Sequential order with most resent-on top
• Any Gaps over 30 days must be explained on the CV
• EPA (Envision Pre-Application) (Please use the attached EPA. It is also in Trio under your Name & then click on Help)
• All Licenses, Boards and Certs with expiration dates must be listed on CV
• Anyone with malpractice must provide a NPDB
• Anyone with legal issues must provide supporting documents

Availability

• 1st date would they be available?
• How many shifts would they be able to do per month?
• What month is their schedule completely open?
• Any shift preferences (Day, Night, Blocksetc.)?
• Any dates in the next 3-6 months they cannot work?
• Any Holidays in the next 3-6 months would they be open to work.?

ABOUT THE FACILITY

• 200-bed facility
• Level I Trauma Centre (State), Level II (ACS)
• ED Volume: 55k; 33 beds
• 14 days of EGS call per month
• Inpatient census: 20/day
• 20 ICU beds, additional 10 medical ICU beds

CERTIFICATION REQUIREMENTS

• ATLS
• BLS
• Board Certified

STATE LICENSE REQUIREMENTS

• Arizona
