It is also in Trio under your Name & then click on Help) All Licenses, Boards and Certs with expiration dates must be listed on CV Anyone with malpractice must provide a NPDB Anyone with legal issues must provide supporting documents 1st date would they be available? How many shifts would they be able to do per month? What month is their schedule completely open? Any shift preferences (Day, Night, Blocksetc.)? Any dates in the next 3-6 months they cannot work? Any Holidays in the next 3-6 months would they be open to work.? 200-bed facility Level I Trauma Centre (State), Level II (ACS) ED Volume : 55k; 33 beds 14 days of EGS call per month Inpatient census : 20 / day 20 ICU beds, additional 10 medical ICU beds ATLS BLS Board Certified Arizona
