Oregon Licensed Practical Nurse

A company is looking for a Licensed Practical Nurse (LPN) with Oregon Licensure for a remote, day shift position.

Key Responsibilities

Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care

Identify and coordinate community resources that would benefit patient care

Provide patient education on managing chronic conditions and perform medication management

Required Qualifications

Active Oregon License (LPN)

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Ability to work in a remote, HIPAA-compliant environment

Experience with Epic is a plus
