🚨 Medical Director – Geriatric / PACE Program
📍 Location: West Springfield, MA
💼 Schedule: Monday–Friday, Full Time
We are seeking an experienced Medical Director to lead a community-based care program for seniors with chronic care needs. This role focuses on serving older adults in the community, helping maintain their independence while ensuring comprehensive, high-quality medical care.
As Medical Director, you will provide oversight of participant care, guide clinical outcomes, and supervise Primary Care Providers (PCPs) and Advanced Practice Providers (APPs). You’ll be a key leader in a collaborative interdisciplinary environment, contributing to the development of medical policies and quality improvement programs.
Key Responsibilities:
- Conduct history and physical exams, develop treatment plans, and provide medically necessary services.
- Participate in intake, assessment, and care planning with participants and caregivers.
- Collaborate with nursing and interdisciplinary teams to optimize patient care.
- Educate participants and families on chronic conditions and health maintenance.
- Supervise and guide PCPs and APPs.
- Manage the healthcare program while ensuring adherence to clinical standards and regulatory requirements.
- Participate in provider on-call rotation as required.

Requirements
- Current, active plenary physician license in Massachusetts – [REQUIRED]
- Board Certified in Internal Medicine or Family Practice – [REQUIRED]
- Board Certification in Geriatrics – [PREFERRED]
- DEA Registration – [REQUIRED]
- Previous supervisory experience – [REQUIRED]
- Minimum 1 year of experience caring for frail or elderly populations – [REQUIRED]
- Strong skills in physical assessment and chronic disease management for older adults
- Ability to work in a dynamic, community-based healthcare setting
