Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. Your Responsibilities

  • Deliver personalized high-quality care to seniors in their homes assisted living hospice and nursing facilities.
  • Conduct comprehensive assessments and develop tailored care plans for older adults.
  • Manage chronic conditions with skill and empathy for patients and families.
  • Collaborate with cross-functional partnersincluding palliative psych and neuroto deliver coordinated whole-person care.
  • Build trusted relationships with residents caregivers and facility partners to enhance engagement and referrals.
  • Maintain consistency in documentation follow-through and patient communication.

    • What Were Looking For

  • Active unencumbered Physician Assistant license in the state you will be practicing.
  • Minimum of 12 years of clinical experience (geriatrics or primary care preferred).
  • Leadership potential or experience : Ideally youve managed a panel successfully and are ready to grow into leadership roles.
  • Experience in home-based care SNF or ALF settings strongly preferred.
  • Compassionate patient-centered approach and strong clinical judgment.
  • Valid drivers license and reliable transportation.
