About The Position

ID : 74503539 Shift : 8hr Days 8x5 Description : HWL Point-AN-0022 Outpatient PT; Shift Monday-Friday 8 : 30a-4 : 45p. Lee Health Outpatient Rehab- Coconut Point- 23450 Via Coconut Point, Estero FL 34135. 2 years of PT experience required. OUTPATIENT Experience required. Active FL license required and must be on profile. BLS required. Large adult outpatient clinic - 74% Medicare population. Patient diagnosis : balance / vestibular, neuro, Parkinson's, neck / back, shoulders and knees. Very busy Adult OP clinic, need a candidate who is well diverse in skill set, Strong teamwork and positive attitude. Epic is used. Prior travel highly preferred. Age group 12-100. HWL clinical prescreen with Manager final interview and offer. \r\n\r\nRadius rule 50 miles enforced. ALL RTO or SCHEDULE REQUESTS must be on submission- HCF will not be approving RTO or schedule requests after submission. Floating required. Please ensure HCP is flexible. Please upload all demographic information to the portal at time of submittal. Travelers are welcomed into the units and are expected to function as core staff does. Hard document deadline is Monday prior to start. No exceptions for pending docs. Guaranteed Hours : 40 Weekly Contract Weeks : 89
