Responsibilities
The Nurse Practitioner (NP) provides comprehensive annual risk assessments in a residential setting or via telehealth, following the scope of practice as delegated by the Collaborative Physician.
Essential Duties and Responsibilities
Conduct thorough in-home health risk assessments to identify active and chronic conditions, and evaluate physical, mental, and social needs at the time of each visit.Gather patient history, perform examinations, and determine diagnoses.Document patient visits as per NCQA standards.Measure and record patient vital signs as necessary.Make case management referrals and liaise with primary care providers (PCPs) when required.Communicate effectively with patients, caregivers, agency nurses, and other providers to ensure appropriate diagnosis and care.Perform all clinical duties while adhering to OSHA Universal Precautions.Ensure patient confidentiality is maintained at all times.Participate in required meetings, in-services, and committees as requested.Engage in professional development activities and maintain relevant licenses and affiliations.In this role you will collaborate with :

Required Knowledge, Skills, and Experience
Active, unrestricted nurse practitioner license to practice.Board certification in one of the following : ANCC, AANP, or NCCPA.Current CPR certification.Active enrollment in Medicare / Medicaid.Valid driver’s license and good driving record.Proficient in electronic health record (EHR) systems.Preferred Knowledge, Skills, and Experience
Experience or training in geriatric care.Ability to work effectively within a team and maintain strong relationships with patients and medical staff.Conditions of this role to be aware of :
Adaptability to varying weather conditions and different residential environments of patients.Ability to perform a full range of physical activity including lifting and handling patients, manual dexterity, normal vision and hearing, standing, bending, walking, and climbing stairs.Ability to regularly lift and carry items weighing up to 50 pounds.Willingness to travel up to 150 miles daily in both urban and rural settings, with the ability to drive if necessary.Pay Transparency
