About the RoleOur recruiting team is hiring
Case Managers to support adults living with
Serious Mental Illness (SMI) across the Phoenix metro. You'll deliver person centered case management, connect clients to care, and support stabilization goals across housing, benefits, and treatment access.
What You'll Do- Manage a community based caseload; complete intakes, assessments, service plans.
- Coordinate care with clinics, prescribers, housing providers, and social service partners.
- Provide crisis intervention, transportation coordination, and appointment support.
- Document timely, compliant notes in the agency's system; uphold confidentiality.
What You'll Bring- 1+ year in case management, behavioral health, or social services (SMI preferred).
- Working knowledge of community resources (RBHAs, housing, benefits).
- Valid AZ driver's license, reliable transportation, and field ready mindset.
