RN Case manager for Penn Medicine Home Health Princeton Health. South team in Plainsboro and West Windsor

Coming from a high volume home health role, will see 5-6 patients per day with high volume of documentation

Required Qualifications

Registered Nurse, Home Health

Licenses : RN(Compact Accepted)

SSN Required

DOB Required

References : 1 Reference in entire work history

2+ years home health experience

Must have case management, OASIS exp to be considered

Preferred Qualifications

Prefer wound care exp as well
