• 8 am - 5 pm Monday through Friday
• 4 - 7 patients per day
• Home health setting
• 1+ years physical therapy home health experience required
• Spanish speaking preferred
• 30 productivity points per week
• SOC assessments and follow-ups
• BLS certification required
