Maryland Licensed Transitional Case Manager

A company is looking for a Transitional Case Manager to facilitate patient transitions from facility settings to post-acute care.

Key Responsibilities :

Educate patients on post-discharge follow-up and homebound criteria

Assess readmission risk and coordinate ancillary services as needed

Liaise between the agency and healthcare providers to ensure continuity of care

Required Qualifications :

Current, unrestricted RN, LPN, or SW licensure in state of practice

Current CPR certification

1+ years of home health or hospital case management experience

Current driver's license and access to a dependable vehicle or public transportation
