[job_card.job_description]The SPCNP's primary role involves offering medical support and coordination to palliative care patients and their families. Key duties include : Providing expertise in pain management, symptom management, and terminal prognosis evaluation to patients, families, and staff. Ordering appropriate therapies and tests as necessary. Conducting in-person and / or telehealth visits based on operational needs. Focusing on patient experience and identifying patient goals of care. Ensuring care aligns with established palliative care guidelines and standards. Regularly communicating and collaborating with HarmonyCares Medical Group and other providers regarding new referrals and ongoing patient care. Coordinating care with external primary care providers and agencies as needed. Engaging with attending physicians about patient care and treatment, working collaboratively with internal partners. Performing patient visits predominantly in private residences, long-term care facilities, assisted living facilities, and hospitals. Providing oversight of pharmacy formulary and medication expenditures. Collaborating with the Chief Medical Officer and Executive Director. Independently commuting to patient visits and participating in marketing meetings as needed. Clinical Staff Providers Patients Patient Facilities Hospitals Patient Family Members Caregivers Agencies Active and unrestricted state licensure as an Advanced Practice Nurse. Valid NPI#, DEA, and CTP. Enrollment with Medicare or state Medicaid or eligibility for enrollment. At least 3 years of relevant experience in home-based or hospice care.

Responsibilities
The Supportive Care / Palliative Care Nurse Practitioner (SPCNP) collaborates with the HarmonyCares Medical Group to deliver comprehensive and quality palliative care services for patients under the direction of the supervising Regional Chief Medical Officer (CMO). Responsibilities include conducting patient visits for both new and established palliative care patients, coordinating their plan of care with designated staff, particularly the palliative care team. Visits may occur in patients' homes, facilities, and hospitals. The SPCNP will independently address healthcare concerns and goals of care, providing palliative services as needed while having access to collaborating providers for consultation.
