Monitor chronic condition patients and track symptoms / vital signsManage medications, address side effects, and optimize symptom control for palliative careEducate patients on disease management, self-care, and end-of-life care optionsDevelop and update individualized care plans, including palliative goals and comfort careProvide health coaching, lifestyle guidance, and emotional support for patients and familiesOffer telehealth services for follow-ups and remote symptom monitoringCollaborate and coordinate with specialists, palliative teams, and hospice providers for integrated careAdvocate for patient access to resources, including palliative and comfort care servicesMaintain accurate patient records and document care plans, including palliative interventionsConduct health risk assessments, with an emphasis on quality of life and symptom managementAddress psychosocial and spiritual needs, facilitating advanced care planning discussionsRequirements and Qualifications
