The Intensive Community Care Nursing Manager (ICCM) is a pivotal role for Registered Nurses (RNs) dedicated to high-complexity patients. In this position, you will collaborate with primary care physicians and other care team members to engage patients in our intensive primary care model, focusing on keeping them healthy and at home while preventing unnecessary hospital visits.

As an ICCM, you will lead the Complex Care Team, conducting assessments, designing individualized care plans, and supervising team efforts to stabilize our highest-risk patients. Your leadership will be crucial in prioritizing cases and guiding your team through the care management process, ensuring safe transitions and outreach to patients in need.

Key Responsibilities :

• Provide in-home, telephonic, and facility visits to high-risk patients, implementing interventions to prevent hospital admissions.
• Conduct comprehensive home assessments, develop care plans, and evaluate patient progress to ensure effective care delivery.
• Supervise team members and coordinate initial assessments to determine patient needs, securing necessary consents.
• Facilitate communication among interdisciplinary teams, ensuring continuity of care and effective service delivery.
• Support patients in navigating healthcare systems, connecting them with community resources, and addressing their holistic needs.
