Job Description :

This is a seasonal position with a tentative end date of May 2026. Employees who remain in their role through the full duration of the assignment will be eligible for an end-of-season bonus. As an RN working on SNP, you will use your clinical expertise to support members with chronic and complex conditions. You will guide, educate, and coordinate care, ensuring everyone receives the right support at the right time.

Your responsibilities may include :

• Conducting health assessments and creating individualized care plans
• Collaborating with care teams to ensure continuity and quality of care
• Helping patients navigate their benefits and care options.
• Providing education and support to promote self-management of chronic conditions
