[job_card.job_description]Physical Therapist (PT) position at WIMIL WFH All Saints Hospital, part of Ascension Health in Racine, Wisconsin. Job not to start prior to 10 / 5. At least 1 year of hospital experience required. May need to float onsite within skill set or to sites within 40 miles if needed. BLS certification required. ARDMS certification required. Completion of Ascension Specialty Exam prior to first assignment and updated every 3 years thereafter in specialty. Must provide documented proof of two MMR vaccines or Positive IGG Titer; no declinations accepted. Fit Test required, preferred to be completed prior to start, can be completed onsite. Documentation of Hepatitis B vaccination series completion or declination signed and dated by traveler. Health Statement or completed physical exam prior to first assignment, updated annually thereafter. Driver's License / State ID must be valid and match address requirements. 2 Step PPD OR IGRA (Quantiferon / Tspot) required within specified timeframes. Documented proof of two Varicella vaccines, serological evidence of immunity, or verification of history of varicella or herpes zoster; no declinations accepted. Tdap vaccination valid within 10 years required. Ascension Skills Checklist to be completed within one year of start; updated annually for specialty. Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually thereafter. Ascension Education Verification within 90 days of start. Ascension Confirmation of Assignment required upon initial assignment and updates upon extension. Ascension Criminal Background Check Results including various verifications within 30 days of first assignment, updated annually thereafter. Ascension EPLS run within 90 days of start, rerun if break in service > Ascension Employment Verification for past seven years, updated annually thereafter. Weekends : 6 weekend days per 13 week assignment. Call : none. Holidays rotating with staff. Forward facing headshot with a white or blue background required; no alterations or filters. Flu season is October 1, 2024 through March 31, 2025; exemptions / declinations accepted but candidate must wear a mask at all times. Qualifying references : at least two professional references prior to first assignment. Post-assignment evaluation may be required. Legal documentation of name change if applicable.
