Position Overview

We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to deliver high-quality nursing care and provide direct supervision to nursing assistants. In this role, you will support our residents' overall health and well-being while ensuring compliance with facility standards and state / federal regulations.

Key Responsibilities

Provide direct nursing care to residents in accordance with care plans and physician orders

Supervise Certified Nursing Assistants (CNAs) during daily care activities

Administer medications and treatments as prescribed

Monitor residents' conditions and report changes to the RN or physician

Participate in care planning, charting, and documentation

Assist in admissions, discharges, and resident assessments

Maintain a clean, safe, and supportive environment

Communicate effectively with residents, families, and interdisciplinary teams

Assist with wound care, Braden assessments, and skin condition monitoring

Maintain accurate medication and narcotic records

Ensure compliance with infection control and safety protocols

Support emergency preparedness and evacuation procedures

Qualifications

Valid New York State LPN license (required)

Graduate of an approved LPN program

Prior experience in long-term care or rehabilitation setting preferred

Strong clinical skills and a solid understanding of nursing practices

Excellent communication, organization, and supervisory skills

Compassionate, team-oriented, and reliable
