Author Health seeks a Licensed Clinical Social Worker (LCSW) to provide clinical care as part of a multidisciplinary team offering remote, virtual-first care to seniors with serious mental illness (SMI) and substance use disorders (SUD). This LCSW will be instrumental in delivering compassionate and comprehensive care to a vulnerable population, leveraging their expertise in mental health to improve patient outcomes. The role requires a deep understanding of the challenges faced by individuals with SMI and the ability to provide tailored interventions through a telehealth platform. This is a part-time position scheduled for 24-29 hours per week : 3-3.5 full days or 5-6 hrs / day, 5 days per week.

WHAT IS YOUR SUPERPOWER?

• Conduct virtual initial assessments and ongoing care for seniors with serious mental illness (SMI) and substance use disorders (SUD), in collaboration with psychiatrists and other healthcare professionals.
• Develop comprehensive treatment plans, which may include coordinating psychotherapy or other treatment modalities, and assessing the need for referrals to specialized care.
• Collaborate effectively as part of a multidisciplinary team to address the holistic needs of patients, including their physical health, behavioral health, and social well-being.
• Provide patient care and education tailored to the individual's age, abilities, educational background, and cultural context, often involving family members in the care process.
• Coordinate care with primary care physicians (PCPs) and other medical professionals to ensure a cohesive approach to patient health.
• Offer timely psychiatric urgent care and be responsive to patients' immediate needs.
• Document all clinical work comprehensively and promptly in the electronic health record (EHR).
• Adapt to a novel care delivery paradigm and thrive in a fast-paced environment.
