A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
Provides outreach to members to support care plan next steps and connect them to health plan and community resources
Serves as the front-line support for member and provider inquiries, explaining care plan procedures and protocols
Documents and maintains non-clinical member records in accordance with state and regulatory requirements
Required Qualifications
High School diploma or GED
1 - 2 years of related experience in healthcare, managed care, or a medical / call center environment
Knowledge of health plan activities and resources
Experience with administrative care management activities
Understanding of Social Determinants of Health (SDOH) needs
