About The Position

Job Title : Home Health Physical Therapist

Experience : Minimum of 2 years of experience required, with home health experience preferred.

License : Must have a valid state license.

Certifications : BLS certification required.

Must-Have :

• Home health experience.
• OASIS training preferred.
• Valid driver's license and car for home visits.
• COVID vaccination or exemption form on file.
• Flu shot or declination, with mandatory mask-wearing while at work.

Description :

This position involves providing home health therapy.

The therapist will conduct skilled visits, typically 6-8 per day.

Minimum of 6 visits per day is required, with potential increases at the manager's discretion.

Patients may include those with cardiopulmonary issues, geriatrics, neurology, orthopedics, and oncology concerns.

Skills required encompass assessment, documentation, and equipment usage, including crutches and gait belts.

Therapists must educate patients and families on signs and symptoms of various conditions.

Safety and injury prevention measures are essential in this role.

The workload will be self-scheduled based on the needs of the home care office.

Documentation will be completed using designated systems.

Interdisciplinary support may be available for patient care as needed.

Candidates may need to float between affiliated facilities.
