Homecare - Wound Care Nurse

Territory : Mahoning, Trumbull, and Columbiana Counties

Business Hours : Monday - Friday, 8 : 00 AM to 4 : 30 PM

This position is paid at a competitive salary with the opportunity to earn extra money by picking up visits at our per diem rate

Mileage reimbursement for any company travel

The responsibilities of our Wound Care Nurses include :

Perform comprehensive wound assessments, including measurement, staging, and documentation of wound characteristics

Develop and implement individualized wound care plans in collaboration with the physician and interdisciplinary team

Administer prescribed treatments and procedures, including wound debridement (as applicable by license and certification), dressing changes, and infection management

Educate patients and caregivers on wound care management, nutrition, skin care, and prevention strategies

Monitor and document wound progression, response to treatment, and any signs of complications

Communicate effectively with the physician and care team to ensure continuity of care and timely updates on patient condition

Maintain accurate and timely clinical documentation in accordance with agency and regulatory requirements

Participate in quality assurance initiatives and wound care performance improvement activities

Ensure compliance with infection control standards and universal precautions at all times

Serve as a clinical resource for staff nurses regarding wound management best practices

All other duties as assigned

Qualifications :

Graduate from an accredited School of Nursing

Licensed in the state as an RN or LPN

Certification in Wound Care (WCC, CWON, CWCN, or equivalent) preferred

Minimum of 1-2 years of clinical nursing experience; home health or wound care experience preferred

Strong knowledge of wound healing principles, wound dressings, and negative pressure therapy (VAC)

Knowledge of Medicare and Medicaid guidelines

Criminal background check

Must have a current CPR certification

Job Conditions :

Valid driver's license required

Ability to access patients' homes, which may not be wheelchair accessible.

Sufficient hearing, eyesight, and physical dexterity for patient assessments and care

Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting / moving / turning of patients
