This position is paid at a competitive salary with the opportunity to earn extra money by picking up visits at our per diem rateMileage reimbursement for any company travelMedical / Dental / Vision Insurance / Supplemental insurance (begins immediately upon hire)Company 401K with 4% company matchingThe responsibilities of our Wound Care Nurses include :
Perform comprehensive wound assessments, including measurement, staging, and documentation of wound characteristicsDevelop and implement individualized wound care plans in collaboration with the physician and interdisciplinary teamAdminister prescribed treatments and procedures, including wound debridement (as applicable by license and certification), dressing changes, and infection managementEducate patients and caregivers on wound care management, nutrition, skin care, and prevention strategiesMonitor and document wound progression, response to treatment, and any signs of complicationsCommunicate effectively with the physician and care team to ensure continuity of care and timely updates on patient conditionMaintain accurate and timely clinical documentation in accordance with agency and regulatory requirementsParticipate in quality assurance initiatives and wound care performance improvement activitiesEnsure compliance with infection control standards and universal precautions at all timesServe as a clinical resource for staff nurses regarding wound management best practicesAll other duties as assignedQualifications :
Graduate from an accredited School of NursingLicensed in the state as an RN or LPNCertification in Wound Care (WCC, CWON, CWCN, or equivalent) preferredMinimum of 1-2 years of clinical nursing experience; home health or wound care experience preferredStrong knowledge of wound healing principles, wound dressings, and negative pressure therapy (VAC)Knowledge of Medicare and Medicaid guidelinesCriminal background checkMust have a current CPR certificationJob Conditions :
Valid driver's license requiredAbility to access patients' homes, which may not be wheelchair accessible.Sufficient hearing, eyesight, and physical dexterity for patient assessments and careStandard nursing activities that include walking, sitting, stooping, standing, and varying lifting / moving / turning of patientsPatriot at Home is an equal opportunity employer
