A company is looking for a Case Manager in Behavioral Health (Crisis Triage) to provide remote support for at-risk members.
Key Responsibilities :
Provides telephone triage, crisis intervention, and care navigation for at-risk members with mental health and / or substance use concerns
Engages with members and families to assess needs and provide resources for timely access to behavioral health services
Matches members with appropriate providers and coordinates care, ensuring effective communication among all parties involved
Qualifications :
Active PA Registered Nurse License required; B.S.N. or LSW, LCSW, LPC preferred
Minimum of three to five years of post-licensure behavioral health clinical experience
Previous experience in Utilization Review or Case Management preferred
Knowledge of DSM 5 and ASAM
Ability to work independently and utilize critical thinking for complex issues
