CMA / RMA or LPN Care Coordinator

A company is looking for a Care Coordinator (Contractor).

Key Responsibilities

Conduct monthly care management calls

Create and update personalized care plans

Identify health barriers and social needs

Required Qualifications

Active CMA / RMA, LPN, or RN with 2-3 years of experience

Experience with EMR / EHR and digital tools

Organized and self-driven

Comfortable with high call volume

Experience in geriatrics or care coordination is a plus
