Job Title : LPN

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience depending on the specialty.

License : Must have license in hand.

Certifications : CPR, BLS

Must-Have :

• Proficiency in HomeCare HomeBase EMR within the last 6 months
• Ability to travel up to 75 miles from the assigned care center

Description :

The LPN provides coordinated and directed home nursing care according to physicians' orders.

Services are based on agency policies and procedures through the application of the nursing process.

The role involves responsibility to a Clinical Manager.

Direct supervision and evaluation of Home Health Aides and Licensed Vocational Nurses are required.

This position requires home visits and attendance at periodic office meetings.

The candidate must have reliable transportation, a current driver's license, and agency-required liability insurance.

The average number of patients seen per day is between 5-8.

The contractor will be assigned between 5-10 patients per day equating to roughly 6-8 points.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Documentation must be completed within 24 hours of each appointment.

There are no guaranteed hours with this contract.

The clinician must hit weekly productivity points of 30, which corresponds to approximately 40 hours per week.

All overtime must be pre-approved.

The clinician must be willing to accept every patient on their schedule regardless of distance.

Delayed and incomplete documentation impacting billing and follow-up patient care may result in cancellations.

Local candidates are accepted at the same rate.

This position does not require on-call duties.

Candidates should be proficient in HomeCare HomeBase and have used it within the last 6 months, or be willing to complete a retraining course if needed.

Candidates should expect to travel frequently within the designated territory.
