JOB PURPOSE
As an Internal Medicine Physician at Heartland Community Health Center, you will be responsible for providing comprehensive primary care to adult and geriatric patients. This role focuses on the prevention, diagnosis, and management of complex acute and chronic medical conditions for patients ages 18 and older.

The Internal Medicine Physician will work within a collaborative, multidisciplinary team that includes Family Medicine, Behavioral Health, Psychiatry, Pharmacy, and Dental Care, ensuring that patients receive coordinated, compassionate, and high-quality care.

DUTIES AND RESPONSIBILITIES
- Provide comprehensive adult primary care, including health maintenance, disease prevention, diagnosis, and treatment of acute and chronic medical conditions.
- Conduct comprehensive medical assessments and develop individualized treatment plans based on patient needs.
- Manage complex chronic diseases, including diabetes, hypertension, COPD, CHF, and other multi-system conditions.
- Perform routine preventive screenings, physical exams, and immunizations in alignment with USPSTF and HRSA guidelines.
- Provide post-hospital and post-ER follow-up care to ensure safe transitions and continuity of care.
- Maintain accurate and timely documentation in the electronic health record (EHR) in compliance with clinical and regulatory standards.
- Educate patients and families on disease management, treatment options, and preventive health behaviors.
- Collaborate with internal team members and external specialists to coordinate patient care.
- Participate in value-based care programs, performance improvement initiatives, and HRSA quality measures to improve population health outcomes.
- Contribute to care coordination and referral management for specialized services.
- Participate in call coverage rotation as assigned (typically one week / month).
- Engage in community outreach, patient education events, and other activities promoting public health and disease prevention.
- Provide mentorship, consultation, and support to Advanced Practice Providers (APRNs, PAs) and clinical support staff.
- Adhere to all Heartland policies, including confidentiality, HIPAA compliance, and professional conduct standards.
- Maintain all required licenses, certifications, and CME participation as defined by state and credentialing requirements.
QUALIFICATIONS
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution.Completion of an accredited residency program in Internal Medicine.Board Certified or Board Eligible in Internal Medicine (ABIM or AOBIM).Valid medical license to practice in the State of Kansas.Valid DEA license.Valid Basic Life Support (BLS) certification.Valid Mental Health First Aid certification (completed upon hire).Prior experience in a community health setting or Federally Qualified Health Center (FQHC) preferred but not required.Demonstrated ability to provide compassionate, trauma-informed, patient-centered care.Proficiency with EHR systems and ability to use data for quality improvement.PHYSICAL REQUIREMENTS
Moves equipment weighing up to 25 pounds.Moves throughout the building to meet with employees, patients, or visitors.Must be able to remain in a stationary position during shift.Ability to read, write, and speak English.Frequent communication with patients about their experience at the health center.
