Job Title : LPN

Hours per Shift : 8 hours

Experience : Minimum of 1 year relevant experience preferred.

License : Licensed and currently registered as a Practical Nurse.

Certifications : BLS or CPR certification required.

Must-Have :

• Completion of LPN program.
• Demonstrates competence and experience in nursing skills within the scope of practice.
• Ability to establish positive team morale and good interpersonal relationships with residents, families, and co-workers.

Description :

Performs tasks and responsibilities within the framework of case finding, provision of supportive and restorative care under the direction of a Registered Professional Nurse.

Performs nursing skills and provides nursing care to an assigned group of residents in accordance with facility policies.

Augments the quality of life of the residents.

Functions as a member of the nursing team and the interdisciplinary team.

Provides direct nursing care to assigned residents with complex needs related to hygiene and physical comfort, medical treatment, nutritional needs, ambulation and exercise, and elimination.

Maintains the safety of residents.

Directs the care of the residents performed by Certified Nursing Assistants.

Administers medication to assigned residents, with some exceptions.

Administers IV therapy with exceptions as indicated.

Prepares residents for diagnostic tests, operative procedures, medical examinations, and transportation.

Admits residents according to facility policies and assists them in adjusting to the environment.

Participates in Person Centered Care planning and coordinates nursing care in the assigned unit.

Contributes to the development and updating of the Person Centered Care Plan to reflect resident needs.

Assists Registered Nurses with health teaching for residents and families as needed.

Establishes positive relationships with residents and families in accordance with the Residents Bill of Rights.

Participates in unit and team meetings.

Observes the nutritional needs of residents and actively participates during mealtime.

Assists in post-mortem care of deceased residents as needed.
