Hospice Social Worker

A Great Opportunity / $53,000-$57,000 / Full-Time

Responsibilities may include :

  • Performing comprehensive patient assessments.
  • Providing patient visits as scheduled.
  • Providing short term crisis intervention and individual or family counseling as needed.
  • Utilizing our electronic medical record system, Brightree, to complete documentation.
  • Serving as a patient and family unit advocate.

Requirements for this position include :

  • Masters degree in Social Work preferred, Bachelors in Social Work accepted.
  • License in Social Work from State of Ohio preferred.
  • Experience in Medical Social Work is preferred.
  • Experience within the Hospice field is preferred.
  • Valid driver's license, car insurance and reliable transportation.
  • Willingness to travel throughout assigned service area.
