About The Position

Case Manager / Utilization Management (90%), Discharge Planning

• National Care Coordination Command Center

UM Develop Plans / Procedures to help improve outcome of reviews

Pilot project to support in Coordination of Care for patients coming out of hospital.

Plans, develops, assesses and evaluates care provided to members. Collaborates with physicians, other members of multi-disciplinary health care team and patient / family in the development. Reviews all inpatient admissions with 24 hours and begins the discharge planning process
