Care Coordinator Specialist

A company is looking for a Care Coordinator - Platform Specialist (Remote).

Key Responsibilities

Collaborate with clinical leadership to develop and document client-specific clinical workflows

Serve as the primary point of contact for on-service providers and address provider pain points

Partner with internal teams to launch new programs and ensure alignment with organizational goals

Required Qualifications

Bachelor's degree in a relevant field

1-2 years of experience in healthcare services, case management, or hospital administration

2+ years of experience in EMR training or clinical systems support

Field-related certifications are a plus

Familiarity with common EMRs used in behavioral health
