We are a growing patient-centered home health agency committed to delivering high-quality skilled nursing care in the home. Our focus is professionalism clinical excellence and supporting our nurses so they can do what they do bestcare for patients.
We are seeking an experienced Skilled Home Health Registered Nurse (RN) to provide comprehensive in-home nursing care to patients in accordance with physician orders care plans and regulatory standards.
Perform Start of Care (SOC) Resumption of Care (ROC) Recertification and Discharge visits
Complete accurate and timely clinical documentation (OASIS skilled nursing notes)
Conduct comprehensive patient assessments and develop individualized care plans
Provide skilled nursing services including wound care medication management IV therapy catheter care and patient education
Communicate effectively with physicians interdisciplinary team members patients and families
Ensure compliance with Medicare state regulations and agency policies
Participate in on-call rotation (if applicable)
Active Registered Nurse (RN) license in the state of Virginia
Minimum 1 year of home health skilled nursing experience preferred
Experience with OASIS documentation required
Strong clinical assessment and time-management skills
Reliable transportation and valid drivers license
Current CPR certification
Ability to work independently and professionally in the field
