Social Worker

Skilled Healthcare Center -

We are a Long-Term care facility seeking an experienced and compassionate Social Worker to join our team. The ideal candidate will have a background in healthcare/ long-term care and a strong understanding of PointClickCare, MDS processes, and community resources.

Responsibilities:

• Provide social services to residents and their families, assisting with adjustment, emotional needs, problem-solving, and financial concerns, including Medicaid applications.

• Gather information from residents and family members to support documentation and care planning.

• Collaborate with residents and families in planning safe and effective discharges back into the community.

• Maintain care conference schedules for both short-term and long-term residents.

• Complete all required Social Services documentation, including MDS sections and progress notes.

• Serve as a liaison between residents, families, and the interdisciplinary care team.

Qualifications:

• Degree in Social Work or a related human services field (required).

• Previous experience in a Long-Term Care or Skilled Nursing Facility (preferred).

• Working knowledge of PointClickCare and MDS documentation (preferred).

• Strong interpersonal and communication skills.

• Admissions or discharge planning experience a plus.
