Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, crafting tailored treatment plans
• Deliver physical therapy interventions targeted at enhancing mobility, strength, balance, and overall functionality
• Work in collaboration with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge planning, and home exercise routines
• Maintain detailed documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards
• Engage in collaborative care team rounds and interdisciplinary discussions to ensure a cohesive care approach

Education

• Doctorate in Physical Therapy (DPT) or its equivalent from an accredited educational institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) may be beneficial but are not mandatory
