Seeking a OT or Occupational Therapist (OT,OTR) to cover therapy services in - Melbourne, FL to provide treatment in an ALF setting : $90 / eval

Responsibilities include but are not limited to :

• Evaluates and treats patients in a Home Setting
• Communicates with families, physicians, and other health care professions
• Maintains daily, thorough documentation of services in the EMR

Qualifications :

Graduate of an accredited college or university with a Doctoral / Masters / Bachelors degree in Occupational Therapy ,

Education and / or Experience : New graduate applicants welcome + Experienced Therapists

Actively Licensed as a FL Occupational Therapist.
