About The Position

Job Title : Physical Therapist Home Health Profession : Therapy / Rehabilitation Specialty : PT Home Health Duration : 13 weeks Shift : Day Hours per Shift : 8 Experience : Minimum 1 full year Adult Home Health Experience License : Must have license in hand Certifications : CPR, BLS Must-Have : Recent Home Health experience Description : Home visits required for patient care. Periodic office meetings will be held. Candidates must have reliable transportation. Current driver's license and agency-required liability insurance are necessary. Average number of patients per day is 5-6. Each type of visit and activity (documentation, travel, etc.) holds a different point value towards productivity. Contractor will be assigned between 5-10 patients per day. Expectations include achieving 30 points per week. Types of visits include Start of Care and Follow-Ups. Weekend requirements will vary and should be discussed during the interview. Candidates must be willing to travel up to 75 miles from the home clinic. Mileage is reimbursed at the IRS Standard rate. The commute to and from the first and last patient is not billable. Candidates must comply with documentation requirements, which must be completed within 24 hours of the appointment. Delayed or incomplete documentation that impacts billing and follow-up patient care will have consequences. There are no guaranteed hours in this contract. Candidates are expected to accept every patient on their schedule, regardless of distance between appointments. If patient load is insufficient, candidates may have the opportunity to float to a nearby clinic to pick up additional patients. Experience with specific electronic medical record systems is required. Candidates must be open to on-call rotations as needed. Must be aware of and comply with all job requirements.
