Texas Licensed Transition Specialist

A company is looking for a Transition Specialist to manage the coordination of member transitions in the community.

Key Responsibilities

Support care coordination teams and develop effective transition plans for members

Assist with member transitions based on care service enrollment and ensure authorizations are honored

Track transition metrics and develop educational materials for members and care coordination staff

Required Qualifications

Bachelor's degree required

2 - 4 years of related experience preferred

Behavioral health experience strongly preferred

Equivalent experience may be considered in lieu of degree

Knowledge of care coordination and community health services is beneficial
