Contract Duration : 14 weeks

Job Description

• Provide diagnostic evaluations and therapeutic interventions for residents with communication, cognitive, and swallowing disorders.
• Develop and implement individualized treatment plans based on patient needs.
• Maintain accurate, timely documentation in accordance with regulatory and facility standards.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Educate patients, families, and caregivers on treatment goals and strategies for communication improvement.
• Participate in care plan meetings and interdisciplinary discussions as required.
• Monitor progress and adjust treatment plans based on resident response and clinical findings.
• Ensure compliance with federal, state, and professional practice guidelines.
• Provide therapy services across two assigned facilities as part of a split role.
