Starting at $25 / hour, guaranteed at least 24 hours per week!

Essential Duties and Responsibilities :

• Ensure quality and safe delivery of hospice services to patients, families, and caregivers
• Perform or assist patients with personal care activities described in the plan of care, including but not limited to hygiene, mobility and elimination. If not able to perform a task, report to the primary RN case manager immediately
• Perform and maintain a safe environment for patients
• Use equipment safely and properly
• Provide universal for conscience and all contacts with patients
• Perform or assist patient / family with home maintenance activities as described in the care plan, including but not limited to light housekeeping and meal preparation
• Perform support activities as prescribed in the care plan including but not limited to supporting independence, providing companionship and providing respite for the family / caregivers
• Respect the autonomy and right to confidentiality of all patients and families / caregivers
• Monitor adequate fluid and nutrition intake, temperature, pulse, respirations, skin and basic elements of body functions and record / report appropriately
• Practice safe transfers, ambulation and normal range of motion and position techniques
• Identify the need for intervention by other members of the interdisciplinary team on an ongoing basis and report to primary RN case manager
• Report on the effectiveness of the care plan
• Document care provided in response with patient and family / caregiver submit documentation in a timely manner per agency policy
• Assist patient, family / caregiver and other team members and providing continuity of care
• Work in cooperation with IDG to meet the emotional needs of the patient's family / caregiver
• Manage a caseload and deliver care as prescribed in the care plan use supplies and equipment effectively and efficiently
• Complete a minimum of 12 hours of in-service per year to include dementia training
• Actively participate in team meetings and contribute positively to the patient's care planning
• Participate in QAPI, special projects and perform other duties as assigned
