Seeking a DPT or Physical Therapist to cover therapy services in - In Freeport, Florida to provide treatment in a home setting : $120 / eval

Responsibilities include but are not limited to :

• Evaluates and treats patients in a Home Setting
• Communicates with families, physicians, and other health care professions
• Maintains daily, thorough documentation of services in the EMR
• Having your own equipment including smart devices and blood pressure monitor.

Qualifications :

Graduate of an accredited college or university with a Doctoral / Masters / Bachelors degree in Physical Therapy ,

Education and / or Experience : New graduate applicants welcome + Experienced Therapists especially Hospice patients

Actively Licensed as a FL Physical Therapist.
