This role provides assessment, care planning, coordination, and ongoing monitoring to help individuals safely remain and receive care in their homes.Key ResponsibilitiesConduct assessments and determine CAP eligibilityDevelop individualized care plans based on identified needsCoordinate services with families, providers, and community agenciesPerform home visits and follow-up monitoringMaintain accurate documentation in E-CAP; review aide notes; support compliant billingEnsure compliance with CAP and state program guidelinesParticipate in required training and continuing educationHybrid Work & TravelBased at the Durham officeEligible for up to 2 remote workdays per week after onboarding and supervisor approvalRegular home visits across Durham County and the surrounding counties (private homes and occasional facilities)Flexible scheduling within program requirementsMust have reliable transportation; mileage reimbursement provided

Requirements :

Education & Experience : Applicants must meet one of the following : Bachelors in Social Work + 1 year related experienceBachelors in Human Services + 2 years related experienceBachelors in Non-Human Services + 2 years related experienceCurrent NC RN license + 1 year case management experienceApprentice / Trainee (Bachelors or RN without full experience)
