As a Clinical Director, you will play a vital role in leading primary care providers (PCPs) at your center. You’ll ensure excellence in clinical quality, patient outcomes, and satisfaction. Alongside your leadership duties, you will also provide direct patient care, ensuring comprehensive support for our older adult patients.

Key Responsibilities :

• Provide holistic care for patients with acute and chronic conditions.
• Take accountability for patient outcomes and collaborate with specialists as necessary.
• Engage with hospitalists for seamless patient care transitions.
• Lead a dedicated care team, ensuring effective management for patients who can visit the office.
• Coordinate with transitional care teams for patients who are hospitalized or homebound.
• Facilitate weekly Super Huddles and Transforming Care Meetings for continuous improvement.
• Support center management and assist in patient recruitment and provider interviews.
• Mentor and coach PCPs to optimize clinical outcomes and team dynamics.
• Collaborate with operational leaders to enhance financial performance and increase center membership.
