As a Clinical Director, you will play a vital role in leading primary care providers (PCPs) at your center. You’ll ensure excellence in clinical quality, patient outcomes, and satisfaction. Alongside your leadership duties, you will also provide direct patient care, ensuring comprehensive support for our older adult patients.
Key Responsibilities :
- Provide holistic care for patients with acute and chronic conditions.
- Take accountability for patient outcomes and collaborate with specialists as necessary.
- Engage with hospitalists for seamless patient care transitions.
- Lead a dedicated care team, ensuring effective management for patients who can visit the office.
- Coordinate with transitional care teams for patients who are hospitalized or homebound.
- Facilitate weekly Super Huddles and Transforming Care Meetings for continuous improvement.
- Support center management and assist in patient recruitment and provider interviews.
- Mentor and coach PCPs to optimize clinical outcomes and team dynamics.
- Collaborate with operational leaders to enhance financial performance and increase center membership.
Skills and Qualifications :
Proficient in Microsoft Office Suite and various healthcare applications.Willingness to travel locally and occasionally at a national level for training and recruitment.Fluency in English, with additional languages welcomed based on the center's needs.Ability to exercise independent judgment in clinical settings.Education and Experience :
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.Active MD licensure required.A minimum of 1-year clinical experience in relevant settings preferred.Completion of specific training and board certification is highly desired.Must possess a current DEA number.BLS certification required within 90 days of employment.At ChenMed, we are dedicated to transforming healthcare for seniors and making a positive impact.
