RN Case Manager

A company is looking for a Case Management Nurse to join their Case Management team.

Key Responsibilities

Assist in the coordination of care across various settings including inpatient, outpatient, and home care

Develop care plans for members facing health challenges and communicate with them to provide education on health conditions

Proactively collaborate with hospital case managers for discharge planning and ensure compliance with applicable laws and regulations

Required Qualifications

Active, unrestricted RN licensure from the United States or a compact multistate unrestricted RN license

Ability to obtain additional state licenses as needed

2+ years of clinical experience in payer, hospital, outpatient, or community-based care management

1+ years of experience in Care Coordination and Navigation

CCM Certification and BSN are considered a bonus
