What You'll Do

• Report significant changes in the patient's condition (physical or emotional), safety concerns, unusual occurrences, and adverse effects of medical devices to the physical therapist or nurse manager promptly
• Utilize physical agents, including but not limited to heat, cold, water, light, electricity, ultrasound, and massage as directed in plan of care
• Observe, record, and report to the Physical Therapist, Clinical Manager, and Health Care Provider, the patient's treatment and respond on a timely basis
• Direct and aid patient/CG in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training
• Provide and document services in accordance with applicable State, Federal, Medicare, Medical regulations and standards and third-party payer guidelines

Qualifications

• Current license as a Physical Therapist Assistant in this state
• At least one year of work experience under the supervision of a licensed physical therapist
• Must have a vehicle, a valid current driver's license, and automobile insurance
• Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively and submit in a timely manner
• Home Health and Home Care Home Base documentation experience preferred
